COFLEX TLC / ANDOVER INTRO:

Andover’s CoFlex TLC Two Layer Compression is a two step technique. Both rolls are latex-free and bond together when applied, which keeps the system in place. The first roll delivers comfort against the patient’s skin. It has a foam layer with a cohesive bandage backing. The second roll is a compression layer and coheres to roll one, stabilizing the wrap and preventing slippage.


GWEN’S INTRO:

Hi, I’m Gwen McComy, I’m the Nurse Manager at Jefferson Regional Wound Care & Hyperbaric Center in St. Louis, MO. I have been practicing for 14 years in this field where I have treated many patients with venous stasis leg ulcers. In the past I have used four layer compression systems as treatments for these patients.

Recently Andover Healthcare requested that I trial their new wound care product CoFlex Two Layer Compression. My patients love the product, they report that it is not as hot on their legs as the four layer systems and is lightweight and comfortable. The staff loves the ease of the application and results.

PRECAUTIONS:

This product should be applied by a licensed healthcare professional. It is not a wound dressing. Please follow wound protocol before applying. Facility guidelines should be followed for infection control.

The CoFlex TLC Two Layer Compression is not recommended for patients with infection, arterial insufficiency, deecompensated heart or diabetes with advanced small vessel disease.

When determining if the patient is suitable for the CoFlex TLC Two Layer Compression, it is important that they have adequate arterial blood flow. Measure the Ankle Brachial index prior to application. If the ABI index is less than 0.8, CoFlex TLC Two Layer compression should not be used.

Patients should loosen or remove the compression system if they experience pain, numbness, tingling, discoloration or swelling of toes. In these instances the product could be wrapped too tightly.

CONSIDERATIONS:

Some patients may not initially tolerate the provided pressure levels of compression. For these patients, CoFlex TLC Two Layer Compression may be applied with less than full stretch.

The CoFlex TLC Two Layer Compression is usually changed one to two times per week or if it becomes loose fitting. The compression system should be kept dry.

APPLICATION – BASE LAYER:

When applying the CoFlex TLC Two Layer Compression be sure that the patient has their foot at a 90 degree angle. This ensures patient comfort and optimal position to wrap with smoothly.

Begin with Roll1, the foam base layer. Apply this layer with the foam side against the skin using a small amount of tension to conform to the shape of the leg with minimal overlap. This ensures a thin application that promotes patient comfort.

Start the application at the base of the toes beginning at the first or fifth digit on the foot at the lateral/dorsal area crossing over the top of the foot.

The next winding runs over the back of the heel. The bottom of the heel will not be completely covered with this layer.

Proceed wrapping up to the knee. Hand-tear or cut any excess material and compress the bandage to ensure it will remain in place during the application of the second layer.

Applying the base layer:

  • Foot at 90 degree angle
  • Begin winding at base of toes
  • Second circle across the top of the foot
  • Next winding over the back of the heel
  • Proceed up to the knee
  • Remove excess material with hand tear or scissors
  • Compress to keep the bandage in place


APPLICATION – TOP LAYER:

Next, use the second roll, the Top Compression Layer. The foot should remain positioned at a 90 degree angle. This should be applied ideally at full stretch but its best to use your judgement with the patient type…if they do not tolerate full compression, apply at slightly less than full stretch.

Start the application just as the first roll was applied, with a circular winding at the base of the toes, again beginning at the first or fifth digit on the foot at the lateral/dorsal area. To ensure even compression, hold the roll close to the leg during application. Cover the heel by wrapping two or three figure eights around the ankle.

Continue to cover the base layer overlapping at 50% and applying with the same compression up the leg.

Hand-tear or cut off any excess material.

Compress the two layers together by gently pressing over the entire application to ensure a strong bond.

RECAP SLIDE:
Applying the top layer
- Position foot at 90 degree angle
- Apply with 75-100% stretch depending on patient
- Begin the winding at base of toes covering the base layer
- Complete 2-3 figure eights around the ankle, cover the entire heel with at least 2 layers.
- Maintain consistent stretch and proceed up the leg with 50% overlap covering the entire inner comfort layer
- Hand tear or cut any excess material
- Press lightly on the entire surface to conform and bond

FOOT WEAR SHOT:
The patient is able to wear normal footwear.

REMOVAL:
CoFlex Two Layer Compression can be removed with bandage scissors or by unwrapping. If using bandage scissors, on opposite side of the wound, lift the wrap from the leg and cut.
